It took a while but it works! (If you’re not running the latest firmware and have previously hacked your iPhone restore with the current version before updating to the latest to avoid getting stuck in Restore mode limbo like I did.) Now available as an automated native iPhone app.

“Only iPhones activated through AT&T are eligible to receive a credit.” So much for “every iPhone customer.” Seems a tad unfair given that the rebate is on the cost of the hardware—not the AT&T contract.
